We make sure all our new patients feel welcome, safe, and taken care of. During your first visit, the doctor will review your medical history. For diagnosis, they will examine you and your health. From there, the doctor will explain any issues and help you choose options for treatment. We will then proceed with appropriate treatment for your care.

To enroll as a new patient with our practice, we would need you to please fill out the New Patient form as seen below and email, fax, or bring it to us in the office. Fill it out to the best of your ability, we need all the information we can get to more efficiently evaluate you upon your visit. We need ALL signatures signed on the paperwork.
